The gauge on the pressure test had a certificate. It ran out eleven days before the test.

The test is unverifiable and has to be repeated — on an insulated, painted, energised system.

Raised in week two, dispositioned verbally, worked off in the field, never written up.

One open nonconformance holds the entire package, however complete everything else is. It is almost always the earliest one.

An as-built that contradicts the log.

A joint gets relocated in the field. It is redlined on the drawing, correctly. The weld log still shows the original.

Both documents are honest, and they disagree. From the outside that is indistinguishable from a records failure.
